Patient Support Programmes
Dialysis care goes beyond the chair. It also includes the medical, nutritional, psychosocial and rehabilitative support that helps patients manage their condition and maintain a better quality of life.
At KDF, we take a holistic approach to care, supporting patients throughout their dialysis journey. With the right treatment and ongoing support, many continue to live long, active and fulfilling lives. Some have been receiving dialysis with KDF for close to 30 years, since our earliest days.
Physiotherapy FIRM Programme
KDF’s Physiotherapy Fall Injury Risk Management (FIRM) Programme was launched in 2023 in response to complications end-stage kidney disease patients face such as muscle wasting and reduced protein reserves that leave them vulnerable and susceptible to falls and injury.
Conducted by our in-house wellness coaches and overseen by a registered Allied Health Professionals Council (AHPC) physiotherapist, the programme features targeted exercises to improve balance, coordination, and muscle strength. Each patient’s programme is carefully designed and individualised to better address each individual’s needs and capabilities.
As part of our continued commitment to providing holistic care to our patients, the FIRM Programme is available to all patients, and we actively review the programme to ensure safety, impact and overall patient experience.
Nutritional Advice from certified dietitian
For dialysis patients, maintaining proper nutrition is essential to health outcomes, yet remains an ongoing challenge. At KDF, we adopt a holistic approach to nutritional support as part of our dialysis care. Patients go through dietitian consultation once every 2 months.
We work closely with dietitians from Food & Nutrition Specialists, who regularly review patients’ blood test results and dietary habits to develop practical, tailored meal plans.
This is complemented by regular educational talks covering a broad range of topics, designed to support patients holistically and empower them to take a more proactive role in safeguarding their health.
Patient Education Seminar and Outing
Established in 1997, Renal Friends is KDF’s dedicated patient support group for individuals living with kidney disease and their caregivers.
It offers a safe and engaging space for ongoing learning, peer support, and social connection.
Through biannual- gatherings and outings, Renal Friends fosters community spirit, empowers patients with healthcare knowledge, and helps them navigate their care journey with greater confidence and resilience.
Vending Hearts
More than just a vending machine, the Vending Hearts Programme allows patients to redeem essential daily necessities up to five times a month, helping to ease cost-of-living pressures while providing added convenience and encouragement throughout their dialysis journey.
Hublet: Information, Education & Entertainment all at once
To enhance the dialysis experience, KDF provides patients with access to Hublet Tablets during their treatment sessions.
These tablets offer a range of educational, informational and entertainment content, allowing patients to stay informed, engaged and connected while undergoing dialysis. Patients can access health education materials, digital resources, videos, and entertainment options that help make treatment sessions more comfortable and meaningful.
By combining technology with patient care, KDF aims to improve the overall dialysis experience and support the emotional well-being of our patients throughout their treatment journey.
